Sunflower Catalog Management Form
The Sunflower Catalog Management Form is used to request the addition, modification or retirement of a catalog entry. For all Sunflower Catalog inquiries, please complete the form and email this form to the DOC Sunflower Help Desk at this email address: SunflowerHelpDesk@doc.gov, and carbon copy or cc your servicing Personal Property Office. Once your request has been processed, they will notify you via email when it is completed.

Sunflower Role Management Form
The Sunflower Role Management Form is used by the Bureau's Property Management Officer (PMO) to request the addition, mofication or retirement of a Sunflower (PPMS) login ID. NOAA Line Offices should complete the form and forward the signed form to the NOAA Chief, Personal Property Management Branch for approval by the NOAA PMO.

Sunflower Organization Management Form
The Sunflower Organization Management Form is used to request the addition, modification or retirement of an organization record. NOAA Sunflower users need to request approval by the NOAA PMO. Completed form should be forwarded to the Chief, Personal Property Management Branch prior to emailing the form to the DOC Sunflower Help Desk at this email address: SunflowerHelpDesk@doc.gov and carbon copy or cc your servicing Personal Property Office. Once your request has been processed, they will notify you via email when it is completed.

Sunflower Location Management Form
The Sunflower Location Management Form is used to request the addition, modification or retirement of a site or building record. NOAA Sunflower users need to request approval by the NOAA PMO. Completed form should be forwarded to the Chief, Personal Property Management Branch prior to emailing the form to the DOC Sunflower Help Desk at this email address: SunflowerHelpDesk@doc.gov and carbon copy or cc your servicing Personal Property Office. Once your request has been processed, they will notify you via email when it is completed.

Sunflower Internal Change Request Form
The Sunflower Internal Change Request Form is used to request a change to Sunflower, categories include Bug Fix, Enhancement and Data Fix. NOAA Sunflower Users need to submit these request forms to the NOAA Chief, Personal Property Management Branch for approval by the NOAA PMO. The NOAA PPMB Chief will submit the request to the DOC Sunflower Help Desk when approved and signed by the NOAA PMO.

CD-50
Personal Property Control - This form is used to request the removal of excess property including furniture and equipment.

CD-52
Report of Review of Property - The form is completed when Capitalized Personal Property is lost, missing, stolen, destroyed, damaged, or rendered unserviceable. ALL accountable personal property must be entered using the CD50 online Sunflower form.

CD-281
Report of Government Property In Possession of Contractor - This form is used by Contractors and grantees annually, or at the completion of a contract or project, to report the inventory of government property in their possession.

OF-347
Order for Supplies and Services - This form is used to establish the formal arrangement between the government and a vendor for the delivery of goods and/or services after a requisition has been approved.

CD-509
Property Transaction Request - This multi-purpose form requests the acquisition, transfer, change and deletion of Capitalized Personal Property and Leased Personal Property. ALL accountable personal property must be entered using the CD50 online Sunflower form.

NOAA Form 37-6
Report of Property Constructed - This form is used to report the completion and acceptance of a piece of property built or constructed for NOAA.

NOAA Form 37-19
NOAA Loan Agreement - This form is used when personal property is loaned to non-NOAA personnel for a fixed period of time.

NOAA Form 37-40
Personal Custody Property Record / Hand Receipt - This form is used whenever it is desirable to ensure that a person is personally responsible for item(s) of personal property.

NOAA Form 37-41
NOAA Heritage Assets Data Compilation - This form is used to report personal property that is being retained by NOAA for its historical, educational, cultural or artistic value.

NOAA Form 37-43
Report of Deferred Maintenance - This form is used to annually report any maintenance of capitalized property that is being deferred.

Optional Form 7
Optional Form 7 Property Pass - This form is used to show authorization for removing personal property from official Government premises.

DD Form 250
Material Inspection and Receiving Report - This form is often used as formal recognition of receipt and acceptance of property ordered under a contract in lieu of the receiving and acceptance report on a OF-347 or any other officially recognized receiving and acceptance form.
